WASCA SPRING FESTIVAL REGISTRATION

AND HOUSING RESERVATION FORM (12/17/09)

Click WASCA 51st Spring Festival Registration and Housing Reservation Form  to download the form in a PDF Fillable Form format.  It may then be completed on your computer.  Please fill the form out completely, and sign in the appropriate places if using a credit card charge. 

Then, print the completed form and submit the two (2) sections separately via the mail as provided below. 

If you want a copy for your records, please print a copy of the completed form.  You may be able to save the completed form to your computer by using Save As and renaming the file, but we have not had consistent results and it may not work. 

How to Fill Out the New Form

The new PDF Form may be completed using Adobe Acrobat.  Simply click on the form and it will download ready to be filled out.  Place your cursor on the first line (Adult #1) and start typing. 

You may be able to Tab to the next field to be filled in.  If Tab does not work, just put your cursor at the beginning of the next blank field (Adult #2 for example) and start typing.

For check boxes, place your cursor over the box and left-click your mouse.  The box will be checked.  If you checked the wrong box, left-click your mouse again and the box will be unchecked. 

How to Submit the Festival Registration Section of the Form 

Registration:  Cut out the Festival Registration Section of the form and mail, along with the proper Registration Fee, to the Registration Chairman (as shown in the Festival Registration Form Section).  Make check payable to WASCA, or provide the requested credit card information (including your signature).  If you have any questions, please contact Joe & Stephanie Corson at (301) 262-3002 or e-mail the Registration Chairman at 2010 FestivalRegistration@wascaclubs.com.

How to Submit the Housing Reservation Section of the Form 

Housing:  Cut out the Housing Reservation Section of the form and mail the completed form, along with a Housing Deposit as shown on the Reservation Form to the Housing Chairman (as shown in the Housing Reservation Form Section).  Make check payable to Hilton Alexandria Mark Center, or provide requested charge card information (Including your signature).  If you have any questions, please contact Jay Kolakoski at (703) 455-3009 or 2010FestivalHousing@wascaclubs.com.
