Click WASCA Promotional Fund Application.doc to download the form in Microsoft Word format.  It may then be completed on your computer.  Please fill the form out completely.  Print the completed form and submit to the Vice President (click for contact information). 

If you do not have Microsoft Word compatible software click WASCA Promotional Fund Application.pdf.  Print that form and complete it by hand and submit to the Vice President, WASCA.

Keep a completed copy for your club's records so that your club will have a history of how the process is done for  your future club leaders.
