WASCA Forms
***We are currently transitioning all of the WASCA forms to a PDF fillable format. The older MS Word format and PDF format versions will be removed from the website. We will start with the WASCA Festival Registration and Hotel Reservation Form. The others will be updated as time permits.***
For all applications, WASCA prefers to receive forms that have been downloaded to a computer and filled in on the computer. To do that download the MS Word format. To print and fill in a form by hand, download the PDF format.***

(Note: The same form is used for both, but the different parts of the form go to different people. The Registration Section goes to the Registration Chairman and the Housing Reservation Section goes to the Housing Chairman.)
