How to print PDF file from your computer using Adobe Acrobat Reader
These are detailed instructions on how to print the form from your computer as an Adobe Acrobat format (.pdf or .PDF) file. Click Adobe Acrobat Reader to download and install the free program on your computer. That will take you to a screen where you can download the program. When you click on the form name, Adobe Acrobat Reader will open and you will see the blank form in a new window on your computer screen. You can click on File at the top left-hand corner of the screen, and a menu will drop down. On that drop-down menu, you will see Print. Click on the Print button. The computer will pop up what is called a dialogue box that is named Print. In the lower right-hand corner of that box you will see a button marked OK. Click the OKbutton. The form will print on your printer.
Once you have printed the form, please complete the form, and make a copy of the completed form for your or your club's records, as appropriate. Submit the completed form with any required payment to the person identified on the previous page. If you need to go back to the previous page, press the green back arrow in the upper left-hand corner at the top of the screen.
