How to download the form as a Microsoft Word document (.doc) file to your computer
These are detailed instructions on how to download the form to your computer as a Microsoft Word format (.doc) file. It will probably be most useful to print these instructions off and then go back to the previous screen. To go back to the previous screen, click on the Back Arrow button at the upper left-hand corner of your screen.
When you click on the form name, you will see the blank form in a new window on your computer screen. You can click on File at the top left-hand corner of your screen. A menu will drop down. On that drop-down menu, you will see Save As. Click on Save As. The computer will pop up what is called a dialogue box that is named Save As. In the lower right-hand corner of that box you will see a button marked OK. Click the OK button.
If your computer has software on it that is Microsoft Word compatible, the computer will save the blank form into a folder that the software maintains. If your computer does not have MS Word compatible software, you will need to download the form in a format that is called PDF. Click How to Print a PDF file from your computer.
After saving the blank form, open the form by going to File and then click Open. Before filling the form out, make sure that you press the key on your computer keyboard called Insert. This key is usually on the right-hand side of the keyboard either at the top or bottom. Then, fill the form out. When you have completed the form, click on File, then Save As (...not just plain Save). At the bottom of the Save As dialogue box, in the blank box following “File name” give the completed form a new name that makes sense to you.
Print the completed form and submit the form with any required payment to the person identified on the previous page. If you need to go back to the previous page, press the green back arrow in the upper left-hand corner at the top of the screen.
