WASCA Forms (new as of 7/22/12)

We are currently transitioning all of the WASCA forms into a fillable PDF format.  As we develop the new forms both the Microsoft Word and older PDF format forms will be removed.

For all application forms WASCA prefers to receive forms that have been prepared on a computer instead of handwritten.  That minimizes errors and reduces the need to follow up.

I want to register for the upcoming WASCA Spring Festival or I want to reserve a hotel room for the upcoming WASCA Spring Festival.

(Note:  The same form is used for both.  The upper section (Festival Registration) goes to the Registration Chairman and the lower section (Housing Reservation Section) goes to the Housing Chairman.
